When calling for your appointment, please let the receptionist know which plan you are covered under. Some plans require a referral form from your primary care physician — please verify with your carrier. If a referral is required, it must be properly filled out and brought to the office at the time of your visit.
If you do not belong to a health plan we participate in, out-of-network benefits usually apply and an out-of-network payment is expected at the time of your visit. We file all insurance claims for you. Please speak to our billing supervisor with any questions on coverage.
